What is in it

The catalog covers a small number of structural failures, each with its own record and sources:

Records withheld from the public. Several distinct records covering matters of significant public interest where material remains sealed, including assassination records still incomplete decades past their statutory deadlines.

Congress exempt from its own laws. Rules applied to the country and not to the body that wrote them.

Settlements paid and hidden. Public money used to settle claims, with the recipients and the circumstances not attributable.

No penalty for agency stonewalling. The structural one underneath several others: an agency that simply does not answer faces nothing, so delay is a complete defence.

Foreign access to classified systems. Access granted to parties accountable to no part of the public.

The economy rigged. The largest and hardest record in the catalog, and the one most in need of sharper root causes.

Institutional capture. Records covering bodies whose independence is the thing that made them worth having.

How to read an entry

Start at the root causes, not the statement. The statement tells you what the movement believes; the causes tell you what could actually be done about it, and which parts nobody has closed yet.

Then check the social-contract violation — that is the link between "we do not like this" and "this breaches the deal", and if it is weak the record is weak.

Then follow the joins: which fix bills claim to close which causes, and which decisions evidence the problem.

The gaps are the useful part

A root cause with no bill against it is the clearest possible statement of where a contribution would matter. The catalog is not a list of complaints; read correctly it is a work queue.

Adding one

Anyone can propose a problem. What makes a proposal strong:

• A named harm with named victims, not a general condition.
• Separable root causes, as many as you can distinguish.
• The structural violation, stated — which commitment is breached.
• What fixed looks like, concretely enough to check.
• Sources.

Approval creates a draft problem record, inheriting the duplicate guard and the frozen key. A decline comes back with a stated reason you can read.
